Special  Recognition  Award

 

REQUIREMENTS

PART  1    -    INFORMATIONAL  

     1)  Letter of nomination by an ATC sponsor

•  State name of hospital or clinic
•  State name of program and year established
• List number of personnel in sports medicine program by

credential.  Also indicate who are members of the IATA
     PART  2    -   CRITERIA

Provide proof of Certified Athletic Trainers employed in the

program (e.g. copy of BOC Certificate and/or ILDoPR
License and proof of employment document)
2)  Provide a Mission Statement (No more than 50 words)

•  Provide a Summary of the History and Services Offered in

the sports medicine program (No more than 100 words)

• List involvement in the IATA

(e.g. CEU sponsorships at IATA meetings, and/or providing
         state educational seminars/workshops/courses)

• List local community involvement

(e.g. ATC services for sports events [little league, fun runs,etc.]
        and/or outreach services to high schools or recreational
        facilities)

• List involvement in GLATA / NATA

(e.g. CEU sponsorships at GLATA / NATA meetings and/or
         providing GLATA / NATA educational seminars/
         workshops/courses)

• List contributions to sports medicine and athletic training

(e.g. research projects, publications, creative/media works,
        product inventions)

• Provide two Letters of Recommendation
• Director or Co-Worker
• Patient or Community Event Organizer

PART 3   -   AWARD  PRESENTED

The Hospital / Clinic Special Recognition Award will be presented at the annual luncheon at the IATA State Meeting and Symposium in November of each year

• The award recipient will receive a plaque, and an honorary membership in the Illinois Athletic Trainers Association, Inc.
