ILLINOIS ATHLETIC TRAINERS’ ASSOCIATION, INC.

SCHOLARSHIP GUIDELINES

                        The scholarships offered by the Illinois Athletic Trainers’
                        Association, Inc. were proposed by the Board of Directors
                        At their meeting October 16, 1994.  Presented to the
                        membership at the annual business meeting in May 1995.
                        These awards will be given to students to perpetuate the
                        standards, ideals and professionalism of Athletic Training
                        in the State of Illinois.  Amended summer 1998, to include
                        an additional offering.

SCHOLARSHIP INFORMATION

1.   Two scholarships of $1000, three scholarships of $500, and two scholarships of $250 are available from the Illinois Athletic Trainers' Association, Inc. They are available on an annual basis but do not have to be awarded annually, depending on the applicant's qualifications. Please note that the applicant must be a member of the IATA at the time of application

2.         The following scholarships are available:

    1. The Dr. Richard Dominguez Memorial Scholarship is an undergraduate scholarship available to a college of high school student athletic trainer who is currently enrolled or has been accepted into an Athletic Training educational program as an undergraduate.  One $1000.00 scholarship is available annually.
    2. A graduate scholarship is available to a college student athletic trainer who is currently enrolled or has been accepted into a post-graduate Athletic Training education program.  One $500.00 scholarship is available annually.
    3. The Dr. John Jevitz Continuing Education Scholarship is available to a licensed member of the IATA, Inc., who is pursuing post-graduate studies on a part-time basis.  Two $250.00 scholarships are available annually.
    4. A scholarship is available to an offspring of a certified/licensed member of the Illinois Athletic Trainers’ Association, Inc.  This student must be currently enrolled in or accepted into a college, or university, regardless of major. Two $500.00 scholarships are available annually.
    5. The IATA Living Memorial Scholarship is an undergraduate scholarship available to a college or high school student athletic trainer who is currently enrolled or has been accepted into an Athletic Training Educational Program as an undergraduate. The applicant must be a student member of the IATA at the time of application.  One $1000 scholarship is available annually.
    6. The Robert C Hamilton award is presented to the student scoring the highest total score on the BOC examination for the previous year. To be eligible for the award the student must be a member of the IATA and have passed the exam on their first attempt. One $1000 award is given annually.
  1. The selection committee consists of the current Vice-President and the Regional Representatives of the IATA Board of Directors.
  1. Application forms are available from the IATA Vice President.  Application requests MUST BE WRITTEN and will be included in the applicant’s application packet.

 

  1. Completed application packets, including all letters of recommendations and transcripts, must be returned to the IATA Vice-President post-marked no later than August 1, 2008.
  1. Scholarships will be awarded by the Vice-President at the IATA State Meeting Awards Luncheon.  Certificates will be awarded to the scholarship recipient and their sponsoring Athletic Trainer, or in the case of the offspring scholarship, the parental member of the IATA.

 

  1. Recipients will be selected based on academic achievement, letters of recommendation and the applicant’s autobiographical sketch.
  1. All applicants must have a grade point average of “B,” or its equivalent, or above.

 

  1. The applicant must submit an official statement from the Registrar of the college, university or high school indicating the applicant’s current GPA and accumulative GPA through the most recently completed school term.  The applicant must be a full-time student at the time of application.  In the case of the Continuing Education Scholarship, the applicant must show proof of enrollment.
  1. The applicant must have three letters of recommendation included in the application packet.  The undergraduate and graduate scholarship applicants should have one letter from the sponsoring ATC, one letter from a professor or teacher in the applicant’s major field of study, and the third letter can be from another athletic trainer, MD, coach or an individual, other than a family member, who can attest to the applicant’s character or other attributes.  The continuing education applicant must have one letter of recommendation from another ATC, athletic director, physical therapist, clinic director, or another professional.  These recommendations should comment on the applicant’s personal character, academic performance and athletic training abilities (when applicable).  The Offspring applicant must have on letter from a professor or teacher in the applicant’s major field of study, one from a community or service organization representative, and one from another individual, other than a family member, who can attest to the applicant’s character or other attributes.

 

  1. All undergraduate and graduate applicants must be a member of the Illinois Athletic Trainers’ Association, Inc.  Applicants for the continuing education scholarship must be a licensed member of the IATA, Inc. for one year prior to application.  The offspring applicant does not need to be a member.
  1. The applicant must mail all documents to the Vice President in one packet.  All documents in the applicant’s packet MUST BE TYPED.  The packet must be post-marked on or before August 1, 2008.

 

  1. If a committee member is a scholarship applicant, he/she will recuse from the evaluation process.  The President-elect or Past-president will gather the committee evaluations.

 

APPLICATIONS FOR THE DR. RICHARD DOMINGUEZ MEMORIAL SCHOLARSHIP (UNDERGRADUATE)

  1. The applicant must be a full-time student and student athletic trainer at the time of application, and must plan on working as a full-time student athletic trainer for the semester that this scholarship is used.  Scholarship checks will be issued to the attending school, upon proof of full-time enrollment for the fall semester after the scholarship is granted.

 

  1. The applicant must be a full-time student, enrolled in an athletic training curriculum leadng to a Bachelor’s degree.

 

APPLICANTS FOR THE GRADUATE SCHOLARSHIP

  1. The applicant must be a graduate degree-seeking candidate for the upcoming fall academic term.  Scholarship checks will be issued to the attending school, upon proof of full-time enrollment for the fall semester after the scholarship is granted.

 

  1. The applicant must have worked at least two academic years as a student athletic trainer on the college or university level under the supervision of an IATA Certified/Licensed sponsoring athletic trainer.

 

 

APPLICANTS FOR THE DR. JOHN JEVITZ CONTINUING EDUCATION SCHOLARSHIP

  1. The applicant must be a licensed member of the IATA, Inc., for one year prior to application, who is currently enrolled part-time at a college of university.  Scholarship checks will be issued to the attending school, upon proof of enrollment for the semester after the scholarship is granted.

 

APPLICANTS FOR THE OFFSPRING SCHOLARSHIP

1.  The applicant must be under the legal guardianship of a parent that is a Certified/Licensed Athletic Trainer and a member of the Illinois Athletic Trainers’ Association, Inc.
       

  1. The applicant must be a full-time student either currently enrolled or accepted into a college or university at the time of application.  Scholarship checks will be issued to the attending school, upon proof of full-time enrollment for the fall semester after the scholarship is granted.
  1. The applicant musts be enrolled as a full-time student at a college or university during the fall term for which this scholarship is used.

 

APPLICANTS FOR THE LIVING MEMORIAL SCHOLARSHIP (UNDERGRADUATE)

  1. The applicant must be a full-time student and student athletic trainer at the time of application, and must plan on working as a full-time student athletic trainer for the semester that this scholarship is used.  Scholarship checks will be issued to the attending school, upon proof of full-time enrollment for the fall semester after the scholarship is granted.

 

  1. The applicant must be a full-time student, enrolled in an athletic training curriculum leadng to a Bachelor’s degree.

 

SCHOLARSHIP RECIPIENTS

Once an applicant has been selected as a recipient of an IATA Scholarship, the recipient:

    1. Will be notified in writing by the IATA Vice-President.
    2. Will be awarded a certificate at the IATA annual business meeting.
    3. Must be enrolled as a full-time student during the next fall term (part-time for the “Part-time graduate scholarship”). 
    4. Must submit, to the IATA Vice-President, an official statement from the institution’s Registrar, indicating student status, for the fall semester following the scholarship award.
    5. Will be awarded the actual scholarship money, after the letter from the registrar is received.
