Dedicated Service Award
Guidelines

The Dedicated Service Award will be given out as a plaque at the Annual Luncheon of the IATA.  An eligible candidate must be an active or retired ATC member of the IATA and an Illinois Licensed Athletic Trainer in good standing for a minimum of 10 years (this includes sitting IATA BOD members elected or appointed).     

Candidates can be nominated up to five times but only receive the Award once.  No more than two candidates will be awarded each year.  Nomination form and associated materials will be due by July 15th to the IATA Vice-President.  Each candidate awarded must have a minimum of 75 points out of 100 points to be considered; higher points  can be accrued.  The IATA Awards Committee will decide who receives the Dedicated Service Award(s) and recommend same to the IATA BOD.

Each Service Award candidate will have an ATC sponsor.  The sponsor will write a nominating letter and solicit from the candidate 1) a nominating letter from a co-worker or administrator; 2) a completed nomination form; and 3) his/her current resume.
